About The Position

As the Palliative Care Unit social worker, you will be an integral member of a dedicated interdisciplinary team providing compassionate care to patients and families facing the end of life. This position is ideal for a proactive professional passionate about Palliative Care and providing high-quality care across diverse patient populations. You will perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct.

Requirements

  • Master's Degree in Social Work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW)

Nice To Haves

  • Palliative Care and/or Hospice Care and Bereavement Experience
  • Mental Health Experience
  • Bi-Lingual Spanish

Responsibilities

  • Provide comprehensive biopsychosocial assessments for seriously and terminally ill patients and their loved ones in the Palliative Care Unit.
  • Establish appropriate plan of care in collaboration with the Palliative Care Unit and hospice teams.
  • Liaise with hospital social workers and teams to facilitate PCU transfers.
  • Provide psychosocial support and counseling to patients and families as part of the interdisciplinary palliative care unit team.
  • Utilize clinical skills including crisis intervention, individual/family counseling, family meetings, psychoeducation, and advocacy.
  • Identify and initiate interventions that alleviate burden and enhance coping for patients and families.
  • Serve as a resource for loved ones who need further support in grief and bereavement, including facilitating bereavement support groups for families and patients.
  • Experience in hospital case management and discharge planning to help facilitate appropriate discharges from the Palliative Care Unit.
  • Provide palliative care education to interdisciplinary colleagues throughout the hospital.
  • Prepare and maintain electronic medical records in accordance with department and/or health system policies.
  • Provide written and oral reports and hand-offs.
  • Occasional coverage for palliative care social work colleagues.
