About The Position
This Social Worker position is part of the Patient Aligned Care Team (PACT) within the Social Work Service at Western New York VA Medical Center in Buffalo. The incumbent Social Worker will be assigned to Outpatient Primary Care, Patient Aligned Care Team, and is a part of and works in collaboration with the extended Interdisciplinary Patient Aligned Care Teams. The PACT Social Worker is responsible for delivery of care, referrals for treatment services and case management services. The Social Worker is assigned to work with a variety of patients including psychiatric, medical/surgical patients and outpatients. Occasional work with inpatients may also be necessary. The incumbent will have full caseload and functions to identify through psychosocial and family assessment, the social, emotional, interpersonal, and environmental needs of the Veteran that may interfere with his/her health, and/or impede the Veteran from obtaining treatment goals. The incumbent is able to exercise appropriate judgement and decision making in triage, assessment, formulation of diagnosis and treatment recommendations.
