About The Position

The Hospice Social Worker (MSW) position at Bethany Hospice and Palliative Care Services is a full-time role focused on providing emotional support and guidance to patients and families during challenging times. Requirements

• Master's Degree in Social Work (MSW) from an accredited institution.
• At least two years of social work experience.
• Good working knowledge of community resources and eligibility requirements.
• Ability to work with state programs and skilled nursing facilities.
• Satisfactory references from employers and/or professional peers.
• Satisfactory criminal background check.
• Presentation of negative TB test or CXR as required by state law.
• Self-directed with the ability to work with little supervision.
• Ability to provide good written documentation in a timely manner.

• Understanding of hospice philosophy and the needs of the terminally ill.
• Flexibility and cooperation in fulfilling obligations.

Responsibilities

• Provide emotional support and guidance to patients and families.
• Work collaboratively in an interdisciplinary setting to meet patient needs.
• Document patient interactions and care plans in a timely manner.
• Assist patients and families in navigating community resources and state programs.
• Maintain compliance with professional standards and practices.
