About The Position
The Social Worker, MSW at MemorialCare Health System is a vital role responsible for providing interventions that address psychosocial barriers to ensure safe and timely transitions through the continuum of care. This position requires a comprehensive understanding of the social, emotional, financial, and cultural factors that affect patients' lives, which are critical in developing effective treatment plans. The social worker will manage a specific caseload of patients, evaluating their needs and collaborating with healthcare teams to provide education, resources, and referrals to patients, families, and caretakers. In this role, the social worker will demonstrate a systematic approach to delivering various therapeutic modalities aimed at removing barriers to care. They will work closely with case managers and other healthcare professionals to ensure that psychosocial and socioeconomic needs are integrated into treatment and discharge plans. The social worker will also facilitate coordination among various services and settings involved in patient care, advocating for patient rights and satisfaction throughout the process. Additionally, the social worker will provide ongoing education to the healthcare team regarding psychosocial issues and the role of social work in patient care. They will manage complex bio psychosocial cases, respond to crises, and ensure that resources are in place to prevent hospital readmissions. This position requires a commitment to maintaining professionalism and a focus on empowering patients to take control of their treatment and decisions regarding their care.
