About The Position

Main Function: Responsible for providing comprehensive assessment and social services interventions and coordination of 24 hour care for CSC participants. Coordinates all matters pertaining to eligibility for CSC services, including verification, or assistance with insurance coverage, including for Medicare and Medicaid, and assistance to a participant or their family member(s) regarding enrollment in CSC. Assesses participants/family psychosocial status and social work needs utilizing professional knowledge, skills for observation and interviewing skills. Assesses a participant's living condition/ situation, cultural influences, and support systems to identify the participant's strengths, and obstacles that affect their living condition in the community. Assesses a participant's need for daily money management annually and as needed. Participates in interdisciplinary team (IDT) meetings. Provides ongoing assessments and services to identified participants/family, including short-term individual and family counseling, community resource planning and crisis intervention. All other duties as assigned.

Requirements

  • Master's degree in Social Work (MSW) after successfully completing a prescribed course of study at a graduate school of Social Work accredited by the Council on Social Work Education and the Education Dept.
  • Minimum of one year of social work experience in a health care setting
  • Minimum of one year of post-master’s social work in a health care setting, preferably with a geriatric population.
  • Minimum of one-year experience with frail elderly population in acute, long term, care or community based setting.
  • In good standing with the Medicare and Medicaid programs
  • COVID vaccinated
