Requirements

  • Master’s degree in Social Work.
  • 2+ years’ experience as a Licensed Social Worker.
  • Licensed Medical Social Worker (LMSW).
  • Skills in critical thinking, communication, customer service, and organization.

Nice To Haves

  • Experience as a Licensed Social Worker in a medical setting.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM).

Responsibilities

  • The Social Worker MSW assesses the social, psychosocial, cultural, environmental, and financial situations concerning the patient and family.
  • He/she is a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate.
  • The Social Worker MSW also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care.
  • Assesses patient/family individual health and psychosocial needs to determine social services and discharge planning needs.
  • Communicates with physicians, staff, and payors to ensure appropriateness of care.
  • Re-evaluates patient needs on an ongoing basis and, as necessary, alters plan of care and for post-discharge care.
  • Actively participates in the interdisciplinary planning process: attends team meetings, identifies resources to provide comprehensive care, and arranges post-discharge care and services.
  • Assesses and communicates the psychosocial health status of the patient/family.
  • Evaluates the plan rendered for effectiveness.
  • Assumes leadership role in the interdisciplinary team to facilitate the development and implementation of the physician’s plan of care for each patient.
  • Reviews all assigned patients for level of care order, including authentication of the order by the MD for all payors and patient classes.
  • Reviews Observation Cases and Extended Recovery patients and communicates with the MD and Utilization Review RN their plan to discharge or required continued stay.
  • Ensures Important Message of Medicare (IMM) is obtained within 2 days of admission and within 2 days of discharge. Understanding of the Appeals process in Compliance with the IMM policy and CMS regulations.
  • Delivers Medicare Outpatient Observation Notice (MOON) to all Observation patients according to the MOON policy and CMS regulation for MOON.
  • Reviews re-admissions and completes the Readmission Flowsheet in EMR. Identifies patients at risk for readmission to enhance education and discharge treatment plan.
  • Establishes a projected discharge plan within 72 hours of admission.
  • Identifies opportunities for the Case Management program to be improved.
  • Assigns avoidable days with appropriate reasons for any patient not meeting MN or delays.
