About The Position

Responsible for coordinating the discharge plan of care for all patients during their hospital admission. Coordinates the discharge plan starting on admission and through discharge follow-up. Provides biopsychosocial assessments and provides updates that address the patient’s evolving discharge care needs to timely move them to the appropriate next level of care. Establishes a plan of care that provides maximum benefit to patients and their families in collaboration with the multidisciplinary team in Care Coordination Rounds (CCRs) seven days a week.

Requirements

  • Master's degree in Social Work is required.
  • Current State of Wisconsin license as a social worker is required.
  • Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients and families.
  • Analytical skills necessary to assess patients needs, develop associated discharge planning and develop solutions to problems concerning patients mental and physical well being.

Nice To Haves

  • A minimum of two years experience in an acute care setting is preferred.
  • Casework in medical social work is preferred.
