Requirements

  • Full-time Social Worker position; At minimum- Master's degree in mental health/social services/counseling field some therapy experience required.
  • Reading and writing consistent with College level. Must have advanced knowledge of medical terminology.

Responsibilities

  • Working with females/males, age range from 6-17 who are developmentally delayed, diagnosed with Autism Spectrum Disorder and experiencing significant psychiatric and behavioral symptoms.
  • Working closely with assigned therapist for continuity of care;
  • Attending facility court hearings and appointments for youth;
  • Participating in weekly treatment team meetings;
  • Managing family visitation schedule;
  • Referring children and families to appropriate services;
  • Developing and implementing appropriate discharge plans with therapist, patient, family and placing agency;
  • Developing positive relationships with external customers, families, patients and employees;
  • Managing transportation requests;
  • Managing treatment planning schedule;
  • Gathering and sending medical records when applicable;
  • Preparing for, schedule and participate in family team conferences;
  • Participating in weekly supervision with the Clinical Program Manager;
  • Establishing and maintaining therapeutic relationships with assigned families, maintain regular contact with agencies/professionals regarding client progress and send monthly treatment plans;
  • Completing intake/biopsychosocial assessments as needed;
  • Maintaining assigned cases;
  • Provide clinical tasks as assigned; individual, group, family therapy, treatment planning, etc.
  • Performing other job-related tasks and duties as assigned
