General Summary of Position The Social Worker works in conjunction with a care team in providing supportive services to patients and their families. Assists Director to ensure all staff is providing quality care to patients and families from admission through discharge and aftercare. Follow and maintains department specific policies procedures and maintains assigned competencies. Primary Duties and Responsibilities Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.Assesses the patient's and family's psychosocial needs as related to the patient's terminal illness.Consults and collaborates with the interdisciplinary team in the development and implementation of the comprehensive plan of care.Documents problems psychococial assessment appropriate goals care provided interventions and patient family and caregiver response to each intervention.Assesses and refers for ongoing bereavement services.Supports the patient unique cultural and spiritual beliefs with referrals to clergy or chaplain as requested.Enhances the responsiveness of the environment by connecting the patient family and caregivers with community resources as needed.Provides interventions for specific symptom relief (fear grief depression anger etc.).Screens for psychopathology and educates/intervenes accordingly.
