About The Position

The Social Worker II (MSW) reports to the Manager of Case Management and Care Transitions. Under guidance of Lead Social Worker, the Social Worker II (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. The Social Worker (MSW) I reports to the Manager of Case Management and Care Transitions. Under guidance of Lead Social Worker, the Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Requirements

  • Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
  • MSW licensure by the South Carolina Board of Social Work Examiners.
  • Requires excellent communication skills, attention to detail, and ability to work independently.
  • Demonstrates ability to collaborate in a positive, professional manner with medical professionals.
  • Requires ability to work in a thorough but efficient manner.
  • Position will cover inpatient, emergency, and Labor and Delivery patients.

Nice To Haves

  • Certification in case management preferred.
  • Acute care experience preferred but not required.
  • Labor and Delivery experience preferred but not required
