About The Position
At CHI Health Midlands we proudly serve Sarpy County and surrounding areas in Nebraska with patient-focused care. Requirements
- Master's Degree from a school of social work accredited by the Council of Social Work Education REQUIRED
- Social Work Certification REQUIRED
Responsibilities
- Offer crisis support, counseling, and resource referrals for patients and families
- Help plan safe discharges and transitions of care
- Coordinate with healthcare teams to keep patients supported and informed
- Connect patients to community resources for housing, food, transportation, and more
