The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities. The Social Worker provides social work services to Hospice patients and families specific to the assignment. This position is responsible for providing clinical services to the terminally ill patient and family, to assist them to cope with the emotional, financial, and practical demands of the illness and the dying process. The Social Worker is responsible for providing interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care. They skillfully advocate in empowering patients to maintain a level of control over their treatment and ensure self-determination. In addition, the Social Worker focuses on ensuring quality care while actively engaging in cost effective measures such as Performance Improvements Projects, LEAN activities, and other hospital wide initiatives. Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
