About The Position

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. The Social Worker is responsible for providing psychosocial services for the team caseload. S/he provides direct services to patient and family, collaborates with the other Interdisciplinary Team (IDT) members within the care team and other health care providers in various care settings. The Social Worker participates in the psycho-spiritual on-call system, as needed. S/he practices in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Hospice of Northern Colorado. Social Worker may be asked to work in all areas of clinical services.

Requirements

  • Graduate of an accredited Master’s Degree in Social Work program
  • A minimum of one year of experience in care setting that delivers end-of-life care
  • Must successfully pass agency required background checks and drug screening
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100K Property Damage coverage) and have reliable transportation
  • Requires travel to multiple work locations
  • Must comply with agency health screening and immunization requirements
  • Must be able to demonstrate computer competency

Responsibilities

  • Identifies the patient’s and family’s psychosocial, environmental, safety and developmental needs and participates in individualizing the patient’s plan of care based on those needs
  • Incorporates patient/family preferences and goals within the plan of care
  • Supports and respects the patient’s and family’s unique spiritual and cultural beliefs
  • Facilitates family meetings
  • Identifies community resources for the patient and families
  • Meets the organizational expectations of a member of a care team
  • Participates in IDT, psycho-spiritual and other patient-related meetings; attends patient care conferences in LTC and AL facilities, as needed
  • Participates in on-call system as requested; this includes evening, night, weekend and holiday hours
  • Supervises students and interns as requested
  • Provides supervision and support for volunteers
  • Knowledgeable of Medicare and Medicaid hospice and home health certification and regulatory compliance requirements
  • Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards
  • Provides patient and family education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members
  • Works collaboratively with volunteers and provides ongoing support and guidance
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely open manner; engages collaboratively in conflict resolution processes as needed
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external
  • Participates in mandatory educational activities and required meetings
  • Is competent using agency information and technology systems
  • Works in a safety conscious manner which insures that safe work practices are used in order not to pose a risk to self or others
  • Models understanding of the organization’s mission and values and fosters cooperative, strong working relationships
  • Presents a professional image by dressing, speaking, and acting professional at all times
  • Supports TRU HNC in the implementation of the Quality Assurance/Performance Improvement Program by sharing information and/or participating in quality initiatives and performance improvement activities
  • Adheres to TRU HNC’s Standards of Conduct and reports any potential misconduct or violation of the Standards of Conduct or any federal or state laws and regulations to the President and CEO or the Guidance Line (1-888-765-7408)
  • Participates in appropriate continuing education as may be requested and/or required by their immediate supervisor, at least 20 hours/year; in addition, accepts personal responsibility for other educational activities to enhance their related skills and abilities; attends mandatory educational programs
  • Abides by TRU HNC privacy practices, complies with the HIPAA privacy and security rules, and reports any privacy or security breaches to the Privacy or Security Officers, respectively
