Summary Provides psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis. Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes.

  • Employs counseling skills in order to help patient/families understand, accept and follow recommendations for care and to restore patient to optimum social and health adjustment.
  • Stays up-to-date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.
  • Intervenes in crisis situations, and effectively applies crisis intervention theory techniques.
  • Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare network.
  • Participates in a monthly or yearly audit or review to ensure adherence to laws and other federal guidelines.
  • Works collaboratively with the patients and patient's social support system to establish and implement a plan.
  • Interviews the patient to complete the initial psychosocial evaluation.
  • Reports suspected cases of child and adult abuse/neglect/exploitation.
  • Master’s Degree in Social Work
  • 1 year experience working as a social worker or applicable mental health experience.
  • Ability to work effectively with people from a broad range of backgrounds.
  • Ability to adhere to the National Association of Social Workers Code of Ethics.
  • Knowledge of patient rights and laws relative to those rights, such as HIPAA.
  • Knowledge of psychological principles and theories.
  • Must possess a high degree of emotional intelligence, and empathy towards others.
  • Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
