About The Position

The Social Worker MSW at Mission Hospital is responsible for conducting non-clinical biopsychosocial assessments and interventions for patients in both inpatient and outpatient settings. This role focuses on crisis intervention, emotional support, and resource referrals, particularly for patients with behavioral health needs. The Social Worker collaborates with healthcare teams to address barriers to discharge and recovery, ensuring patient-centered care that respects individual preferences and values.

Responsibilities

• Conduct non-clinical biopsychosocial assessments and interventions for inpatient and outpatient emergency department patients.
• Perform crisis intervention and provide focused emotional support to patients and families.
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual factors.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and processes.
• Develop appropriate discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care teams and patients/families/caregivers to facilitate decision-making.
• Provide emotional support during complex issues such as end-of-life notifications and crisis interventions.
