About The Position

The Social Worker MSW at HCA Healthcare is responsible for providing non-clinical, biopsychosocial assessments and interventions for patients in inpatient and outpatient settings. This role focuses on crisis intervention, emotional support, and resource referrals, ensuring patient-centered care that respects personal self-determination. The Social Worker collaborates with healthcare teams to address barriers to discharge and recovery, particularly for high-risk patients, and engages in compassionate problem-solving to meet the diverse needs of patients and their families.

Requirements

  • Masters Degree in Social Work (MSW) or equivalent.
  • Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW).

Responsibilities

  • Perform non-clinical biopsychosocial assessments and interventions for patients.
  • Conduct crisis intervention and provide focused emotional support to patients and families.
  • Assist in post-acute care placement and resource referrals for patients with behavioral health needs.
  • Collaborate with RN case managers, nurses, physicians, and insurance companies to address patient needs.
  • Identify and address barriers to discharge, including financial, emotional, and psychosocial factors.
  • Maintain knowledge of Medicare, Medicaid, and private payer regulations.
  • Develop discharge plans for high-risk patients with psychosocial needs.
  • Facilitate meetings between care teams and patients/families to assist in decision-making.
  • Provide emotional support during complex issues such as end-of-life notifications and crisis interventions.
