About The Position

The Social Worker (MSW) in Urgent Care at Kaiser Permanente provides essential clinical social work services in a primary care or home health setting. This role focuses on supporting the care team in managing patients' psychosocial needs, enabling them to maintain independence, and coordinating care plans. The social worker will act as a clinical resource for the Urgent Care teams, addressing mental health, substance use, and psychosocial health needs, particularly for patients in acute crisis.

Requirements

  • Minimum one (1) year of previous social work experience.
  • Masters degree in social work (MSW).
  • Licensed Advanced Social Worker License (Washington) OR Licensed Independent Clinical Social Worker (Washington) OR Licensed Social Worker Associate-Independent Clinical (Washington) OR Mental Health Counselor License (Washington) OR Licensed Marriage and Family Therapist (Washington).
  • Excellent written and verbal communication skills.
  • Demonstrated success as part of a multi-disciplinary team and commitment to diversity.

Nice To Haves

  • Two (2) years of post-masters experience.
  • Care coordination experience in managed care system, in-/outpatient setting, skilled nursing or long-term care facility.
  • Knowledge of social case management, conflict resolution, and team building practices.
  • Familiarity with Microsoft Office.
  • Familiarity with electronic charting.
  • Emergency Department or Crisis experience preferred.

Responsibilities

  • Provide appropriate social work interventions to all age patients and their families, including psychosocial assessment, advocacy, linkage, referral, and long-term care planning.
  • Consult and coordinate with a multidisciplinary team of care professionals and primary care site medical staff, including family and care conferences.
  • Assess patient and family needs for care and communicate with other healthcare providers appropriately, collecting relevant data throughout the duration of care.
  • Evaluate psychosocial assets and deficits to enable patients to maintain the greatest level of independence.
  • Modify the plan of care based on ongoing assessment and patient/care systems response.
