The position involves providing emotional support and practical assistance to patients and their families, conducting assessments of patients' psychosocial needs, and educating them about illnesses and treatment plans. The role includes counseling those in crisis or experiencing distress, assessing the financial status of patients and families, and connecting them with other facilities and resources. Maintaining the confidentiality of patient records and evaluating a patient's mental health are also key responsibilities. The position requires effective communication and collaboration with all members of the health care team to ensure that patients receive services in a safe and timely manner, and that they and their families are provided with all necessary resources and education. The role also involves interpreting and communicating complex patient/family needs and role modeling caring practices to other caregivers. Additionally, the position requires assessing psychosocial risk factors, counseling patients and families regarding the emotional, social, and financial consequences of illness, and providing mental health assessments upon physician referral. The candidate will also provide therapeutic intervention for complex discharge planning and serve as a liaison between Indiana University Health and state and county protective teams.
