About the position

HCA Florida Fawcett Hospital is seeking a Social Worker MSW PRN to ensure high quality, efficient care for patients. The Social Worker (MSW) will be responsible for non-clinical, biopsychosocial assessments and interventions for inpatient floors and the outpatient emergency department. This role includes crisis intervention, patient/family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resource needs. The MSW will assist in post-acute care placement on all discharges, particularly for patients with post-discharge behavioral health and resource needs. The position requires collaboration with RN case managers, acute care nurses, physicians, and other staff to address patient needs and barriers to discharge and recovery.

Responsibilities

  • Collaborate with RN case managers, acute care nurses, physicians, and insurance companies to identify and address patient needs.
  • Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual.
  • Maintain knowledge of Medicare, Medicaid, and private payer regulations and processes.
  • Perform thorough assessments and develop discharge plans for high-risk patients.
  • Coordinate and facilitate meetings between the care team and patients/families/caregivers.
  • Provide emotional support to patients and family members during complex issues such as end-of-life notifications and crisis interventions.

Requirements

  • Masters of Social Work (MSW) required.
  • 1+ years' experience or MSW internship required.
  • Licensed Clinical Social Worker preferred.
  • Certification in Social Work preferred.
  • Critical thinking skills, service excellence, and good interpersonal skills.
  • Ability to read and comprehend written/verbal instruction.
  • PC Skills.
