Good Shepherd Hospice, a member of Catholic Health, is seeking a compassionate, dedicated Social Worker to assist patients receiving hospice care and their families in coping with problems resulting from terminal illness. In doing so, the social worker interviews patients and families to obtain psychosocial history and, in conjunction with Hospice team members, establishes a comprehensive, interdisciplinary plan of care. Prepares and updates social work case records including patient and family assessments, nature of social problems, therapy, intervention and response. Directs patients to designated community agencies or resources as necessary. The ideal candidate will have knowledge of social work theories, therapies and techniques of casework process, as well as principles of public welfare and must possess the ability to work non-judgmentally with patients and families of many cultural, religious and socioeconomic backgrounds and lifestyles.
