About The Position

This position at Dartmouth Health involves providing social work services with a focus on care management. The role includes weekend coverage with 2-10 hour shifts and the possibility of supporting 8 hours of Monday psychiatry coverage. The social worker will be responsible for conducting assessments, offering counseling, and providing advocacy services to patients, either directly or through consultation.

Requirements

• Master's Degree in Social Work (MSW) required.
• Social work experience in an acute, home care, or ambulatory setting preferred.
• Strong organizational skills, verbal and written communications skills.
• Sound decision making and judgment.
• Time management skills.
• Negotiation skills.
• Proven ability to deal with conflict resolution.
• Comfortable with basic computer skills.

Responsibilities

• Provides complete psychosocial assessments of patients and families.
• Provides counseling to individuals and families to help solve social and emotional problems.
• Coordinates community resources to address/meet identified needs.
• Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
• Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
• Documents objective, concise and pertinent clinical data in the medical record.
• Performs other duties as assigned.
