About The Position

The Social Worker MSW / LCSW provides face-to-face assessments, counseling, discharge planning, and crisis intervention for patients and families. This role involves collaborating with the healthcare team to ensure safe, timely, and effective care while addressing complex psychosocial and safety needs. The social worker also serves as a key resource in abuse/neglect cases, guardianship, foster care, adoption, and mental health placements, supporting patients through crises with de-escalation and clinical leadership.

Requirements

  • Master’s Degree in Social Work from an accredited institution.
  • Permanent Indiana LCSW license required.
  • Certification in Crisis Prevention Intervention or an equivalent de-escalation training within 6 months of hire.
  • 3-5 years of relevant healthcare, mental health, or community-based experience preferred.
  • Evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention.
  • Understanding of disease processes and human behavior related to health social needs.
  • Complex discharge planning skills.
  • Knowledge of state and county legal mandates for child and adult protective services.
  • Principles of growth and development across the lifespan, including end-of-life care.
  • Ability to identify decision-makers and facilitate goals of care conversations.
  • Experience working with geographically, culturally, and financially diverse populations.
  • Strong interpersonal, communication, and collaboration skills.
  • Proficiency in electronic medical records and documentation.

Responsibilities

  • Meet face-to-face with patients and families for psychosocial assessment and counseling.
  • Assist in developing educational materials for team members, patients, and families.
  • Serve as a resource and role model for students, interns, LBSWs, and other social workers.
  • Participate with the clinical team to develop treatment plans that address psychosocial issues and needs.
  • Act as a consultant for providers regarding mental health diagnoses.
  • Collaborate effectively with all members of the healthcare team to facilitate safe and timely care delivery.
  • Interpret and communicate complex patient/family needs; role-model caring practices.
  • Assess psychosocial risk factors, including prior functioning, support systems, reactions to illness, and coping mechanisms.
  • Counsel patients and families regarding emotional, social, and financial impacts of health conditions.
  • Access and mobilize community and family resources to meet patient needs.
  • Conduct mental health assessments upon physician referral.
  • Facilitate complex discharge planning and therapeutic interventions.
  • Serve as the primary contact in cases of child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence.
  • Act as liaison with Indiana University Health and local child and adult protective teams (APS/DCS).
  • Maintain principles of crisis intervention and de-escalation, supporting safety in acute situations.
  • Provide intervention support for guardianship (temporary and permanent), foster care, adoption, and Level II mental health placements.
  • Deliver specialized mental health services, including crisis intervention, addiction assessments, and psychiatric referrals.
  • Support individuals in crisis, utilizing de-escalation techniques to prevent violence.
  • Conduct interviews with victims, perpetrators, and significant others, applying effective interviewing principles.
  • Provide intervention for end-of-life, guardianship, foster care, and adoption cases.
  • Build and sustain strong working relationships with hospital leadership, law enforcement, community agencies, schools, probation, and other stakeholders.
