Social Worker MSW - Inpatient Case Management
About The Position
The Social Worker MSW is an excellent opportunity at MultiCare, responsible for aiding participants and their families with personal and environmental difficulties that predispose illness or interfere with remaining in their home environment. In this role you will be able to apply your strong analysis, interpretation and interpersonal skills to support patients through complex issues. In addition to direct patient care, this role encompasses responsibilities for training, mentoring, and precepting new team members, students, and interns as delegated, supporting the ongoing development of a high-performing and compassionate care team.
Requirements
- Master's Degree in Social Work
- LSWAIC, LMHC, LMFT, LASW, LICSW as required by department
- Experience working in a hospital/acute situation or equivalent social service agency, experience relative to the medical social work function
- Depending on assigned area, BLS Certification may be required
Nice To Haves
- Experience in training, mentoring, or precepting new staff or students is highly desirable.
Responsibilities
- Administer assessment tools and gather data for assessment, and to assess for optional needed services
- Interview families to determine social work needs of the participant
- Contact community resources to coordinate services
- Provide initial and on-going assessment of participant's social, emotional and cognitive status and needs, involving participant and other staff members in the assessment
- Ensure accurate documentation is maintained for all assessments, treatment plans, and evaluations
- Serve as a preceptor for new social workers, interns and students, providing guidance, mentorship, and support during onboarding and training.
- Facilitate training sessions and contribute to the development of educational materials to enhance staff competencies in case management and psychosocial care.
- Collaborate with leadership to identify training needs and implement best practices in social work and emergency department case management.
