The PEM Care Management department is made up of a diverse group of individuals. Each come with their own unique backgrounds, skills, and experiences. The department is comprised of Social workers, RNs, and CMAs. Together we have built a team that can draw on each other’s strengths and supplement each other’s weaknesses. We expand each other’s knowledge and skills through collaboration and coaching, and provide opportunities for growth within the department and organization. We work together as a team every day to expedite the most appropriate and timely discharges for our patients. We work collaboratively with other members of the interdisciplinary team, as well as post-acute providers to ensure continuity of care. We do this and so much more everyday as a team! Responsibilities This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.

Requirements

  • Master’s degree in Social Work from an accredited social work program Required
  • 1 year of experience in an acute or post-acute setting Required
  • LMSW - Licensed Medical Social Worker - State Licensure Preferred

  • Accredited Case Manager (ACM)
  • CCM - Certified Case Manager
