Join a team dedicated to providing compassionate, comprehensive end-of-life care. As a Social Worker in Family Hospice, you will support patients and families through some of life’s most challenging moments. You’ll collaborate closely with nursing, physicians, nursing assistants, and spiritual care to meet intense psychosocial needs, guide families through complex grief, and help create meaningful legacy projects such as fingerprint cards, hand castings, veteran ceremonies, and nursing honor ceremonies. This role is not just about providing social work services—it’s about shaping the final chapters of life with dignity, compassion, and creativity. You’ll have the support of an extraordinary interdisciplinary team, the opportunity to innovate with legacy projects, and the chance to guide families through profound moments of love, loss, and remembrance. Full-time benefits.
