About The Position

The MSW Social Worker in Hospice is responsible for providing comprehensive social work services to hospice patients, families, and caregivers. This role focuses on addressing psychosocial issues related to end-of-life care, including adjustment to illness, care system navigation, and end-of-life planning. The social worker collaborates with the Hospice Interdisciplinary Group to develop and implement care plans, ensuring continuity of care and enhancing the quality of life for patients and their families.

Requirements

• Master's Degree from an accredited School of Social Work.
• Washington Advanced Social Worker License upon hire.
• Washington Advanced Social Worker Associate License upon hire.
• Washington Clinical Independent Social Worker License upon hire.
• Washington Clinical Independent Social Worker Associate License upon hire.

Nice To Haves

• 1 year of Post-Masters work experience.
• 1 year of prior internship experience in a Providence Health System setting.
• 1 year of prior experience in a healthcare setting.

Responsibilities

• Travel to see patients primarily in the northern Seattle, Kenmore, and Bothell area.
• Provide services to hospice patients, families, and caregivers.
• Address a wide range of social work care coordination and psychosocial issues related to end-of-life needs.
• Develop and implement a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG).
• Manage an assigned caseload shared with an RN Case Manager and a Hospice HA.
