About The Position

The Hospice Medical Social Worker at AdventHealth is responsible for delivering various social work services to terminally ill patients and their families. This position provides initial psychosocial evaluation, ongoing psychosocial support, direct casework services, and referral to community resources to assist with end-of-life care needs. Additionally, this role collaborates with the hospice interdisciplinary team to implement, evaluate, and adjust individualized plans of care. The position also assists with Medicaid applications, Nursing Home and ALF placements, hospice inpatient unit placements, and financial needs. Furthermore, this role engages in community education and outreach, aiming to support patients and families in reaching desired outcomes through effective social work interventions.

Requirements

• Master's degree.
• 1 year experience in health care setting.
• Active American Heart Association BLS.
• Valid in state Drivers License with current car insurance.

Nice To Haves

• EMR experience.
• Hospice experience.
• Hospice Medicare Conditions of Participation.
• Clinical Social Worker License (LCSW).

Responsibilities
