Social Worker-MSW HIV Grant
About The Position
The Social Worker-MSW HIV Grant provides assistance to HIV patients and their families in the form of diagnostic evaluation; individual, family & group therapeutic services; crisis intervention, and resource referral. Identifies patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge. Works collaboratively with other members of the interdisciplinary team in developing and implementing a comprehensive, integrated discharge plan. Makes appropriate referrals for community services; may provide inpatient emotional support and counseling services, crisis intervention, and bereavement services. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence. Provides care and services to all patient groups: Neonatal, infant, pediatric, adolescent, adult and elderly. Performs other duties as needed.
Requirements
- Master's degree in social work required
- One year of experience in clinical setting required
- Able to read; write legibly; speak in English with professional quality
- use computer, printer, and software programs necessary to the position
