DESCRIPTION OF POSITION: The Clinical Case Management Social Worker performs Social Service assessment and addresses counseling needs. SUPERVISION: Reports directly to Director of Clinical Case Management RESPONSIBILITIES AND DUTIES: Develops and maintains thorough knowledge of referral resources throughout the service area. Facilitates home health and nursing home placements and transfers. Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies and others. Practices proactive discharge planning and coordination of care. Maintains accurate and timely records, including clinical and progress notes, according to department policy. EDUCATION AND WORK EXPERIENCE Master's in Social Work, Case Management certification preferred. One (1) year of social work experience in a health care setting. Computer skills at data entry level. Must maintain required hours of in-service per year to meet requirements for CEU's as stipulated by the Louisiana State Board of Social Work Examiners (20 hours). Ability to communicate with patient, family peers and medical personnel and community referral agencies. Physical Demands/Work Environment Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time. Must be able to respond quickly and effectively to emergency and non-emergent situations. May be required to assist in controlling disorderly conduct or combative patients. Must be able to exchange accurate information with patient, family, peers and medical personnel.
