About The Position

The Social Worker at AdventHealth plays a crucial role in addressing the complex psychosocial needs of patients. Requirements

• Current degree as a Master-level (MSW) Social Worker in applicable state.

Nice To Haves

• Accredited Case Manager/Certified Case Manager certification.
• Current, active registration with the applicable State Department of Health as a Licensed Clinical Social Worker (LCSW)

Responsibilities

• Intervene with patients who have complex psychosocial needs.
• Assist with eligibility determination for social programs and funding sources.
• Provide crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of a discharge plan of care for high-risk patient populations.
• Receive referrals for individuals from at-risk populations from interdisciplinary team members.
