Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. Conducts social work visits and care coordination; documents in medical record as appropriate. Performs additional follow-up visits as indicated or determined by clinical professionals involved with care; documents as appropriate. Establishes and maintains a supportive professional rapport with patient, family, physicians and other members of the interdisciplinary team. Provides patient and family education on community resources and reinforces the plan of care with education, counseling and family/patient conferences as needed / ordered by physician. Evaluate patient and family response to treatment/plan of care and education. Helps patients understand their diagnosis and make the necessary adjustments to their lifestyle, housing, or healthcare. Assess clients’ needs, situations, strengths, and support networks to determine their goals.
