HOW YOU'LL MAKE AN IMPACT Your Role: Works as a multi-disciplinary team member. Assesses and treats patients/clients/families with emotional/behavioral/health problems. Provides social work services to patients/clients/families with behavioral, social, financial/economic, and environmental problems. Interviews patients/clients and their families. Evaluates, and understands the social and emotional factors underlying patients’/clients’ illnesses; identifies their psychological needs, goals, and treatment preferences; coordinates a range of services; and facilitates individualized case management. Provides clinical counseling and individual therapy to help patients/clients/families make positive behavior changes. Assists patients/clients in treatment planning. Provides direction to clinical team for implementing treatment plans. Provides crisis intervention services to patients/clients and families. Advises family caregivers; provides case and care management or interventions designed to promote health/well-being and prevent disease. Provides patients/clients/families with information and counseling; makes referrals for other services. Mains contact with patients/clients/families to assist in adjusting their needs through use of medical facilities or referral to outside supportive agencies. May execute discharge planning responsibilities. Participates in social and health programs in the region/community; may assist/participate in planning supportive events. May travel to patients’/clients’ homes to render services. Displays an understanding of diversity. Demonstrates integrity, compassion, accountability, respect, and excellence (I CARE values). Enhances and contributes to a safe environment for all colleagues, visitors, and patients. Performs miscellaneous duties not expressly described herein.
