About The Position

The position involves completing psychosocial assessments for patients post-hospital care, assisting them in coping with stress related to hospitalization, disability, and chronic or terminal illness. The role requires collaboration with the Clinical Care Management team to evaluate patients for appropriate care settings and to serve as a resource for hospital staff and physicians regarding the emotional and social aspects of patients' illnesses. Additionally, the position includes providing education on community resources, making referrals, and responding to consultative service requests after hours.

Requirements

  • Masters in Social Work required, LCSW preferred.
  • Applicable state certification is preferred.
  • Adherence to National Association of Social Workers Code of Ethics.
  • At least 1 year of professional experience in a hospital or health-related setting preferred.
  • Expertise with Data Management Tools.

Responsibilities

  • Completes psychosocial assessments for designated patients and their support systems.
  • Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
  • Works with the Clinical Care Management team to assess and evaluate the patient for appropriate care levels.
  • Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient's illness.
  • Provides information and education to patients on community resources and options for post-hospital care.
  • Makes referrals to community agencies as needed.
  • Reports suspected cases of child and adult abuse/neglect/exploitation.
  • Serves as a liaison between the hospital, patients, and the Department of Social Services during evaluations/investigations.
  • Responds to requests for consultative services to patients after normal working hours.
