About The Position
Silicon Valley Adult Day Health Care Center (SV ADHC) is seeking a dedicated and compassionate Social Worker (MSW) to join our team. This full-time position is based at our facility located at 631 S Milpitas Blvd, Milpitas, CA 95035. The ideal candidate will be responsible for providing comprehensive social services to our participants, which includes conducting assessments, developing care plans, and facilitating participant education. The role requires a strong commitment to supporting the health and well-being of our participants, many of whom are elderly or have chronic health conditions. As a Social Worker at SV ADHC, you will engage in various duties such as participant assessment and care plan development, new participant intake, home evaluations, discharge planning, and providing both individual and group support. You will also be responsible for educating participants about available services and resources, ensuring they receive the necessary support to thrive in their daily lives. This position requires a collaborative approach, working closely with other healthcare professionals to ensure a holistic approach to participant care. The successful candidate will possess a Master's degree in social work or a related field, along with relevant experience in social services, particularly in aging, health, or long-term care. Bilingual proficiency in Mandarin or Cantonese is highly desirable, as it will enhance communication with our diverse participant population. This role is essential in fostering a supportive environment that promotes the dignity and independence of our participants.
