About The Position

Requirements

• Master's degree in Social Work
• 1 year of experience in social work
• Lic Clinical Social Worker (LCSW) or Lic Masters Social Worker (LMSW) or Lic Master Social Wrk AdvPrac (LMSW-AP) certification through the state of TX

Responsibilities

• Conduct and document detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient.
• Employ advanced practice methods in the development, implementation, and evaluation of treatment plans.
• Provide patient and family education and serve as a professional resource.
• Supervise plan of care interventions including resource and referral help, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate.
• Partner with the healthcare team and involve the patient and family in the development and implementation of plans.
• Serve as a liaison with community resources to ensure smooth transitions and placements for patients.
• Collaborate with other healthcare team members to achieve positive patient outcomes.
• Participate in performance improvement initiatives including data collection and review, and development of continuing education programs.
• Prepare and write professional reports, letters, and case summaries.
