About The Position

The Mental Health Rehabilitation Specialist provides direct members served community support and case management services to assigned members served using Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices. This position provides education and support to members served and their families. S/he coordinates service needs and collaborates with other services and agencies such as the Conservator's Office, Public Defender's Office, Child and adult Protective Services, Probation Department, and Inpatient Services, Long Term Care Services, Residential Settings and County Mental Health Services. The Mental Health Rehabilitation Specialist completes assessments, provides mental health rehabilitation services, case management, crisis intervention, integrated whole person care, and trauma informed care.

Requirements

  • Master's degree in Social Work, Psychology, Counseling, Human Services or Social Services and two (2) years of experience providing direct services in Mental Health; or
  • Bachelor's degree in Social Work, Psychology, Counseling, Human Services or Social Services and four (4) years of experience providing direct services in Mental Health; or
  • Associate's Degree and six (6) years of experience providing direct services in Mental Health.
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups.

Responsibilities

  • Provide direct community support and case management services to assigned members served.
  • Utilize Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices.
  • Educate and support members served and their families.
  • Coordinate service needs and collaborate with other services and agencies.
  • Complete assessments and provide mental health rehabilitation services.
  • Conduct case management and crisis intervention.
  • Deliver integrated whole person care and trauma informed care.
