About The Position
Regional West Health Services is an independent, integrated health care delivery system based in Scottsbluff, Nebraska that is seeking talented Social Worker or Medical Social Services Representative. As an RWHS Social Worker or Medical Social Services Representative, you will: Provides objective casework services to patients and their families which include: social assessments, problem identification, evaluation, planning, referral, education and counseling related to social, financial and economic issues Coordinates discharge planning in collaboration with the health care team, patients and or families and resources within the community. Communicates with physicians, ancillary staff and services as a resource to the healthcare team.
Requirements
- Degree in Social Work or related field
