About The Position

Kline Galland Home is actively seeking a Social Worker (Discharge Planner) for a well-established team on our Transitional Care Unit (TCU) where patients receive short-term, post hospitalization rehabilitation. The social worker will play an active role as a member of an interdisciplinary team consisting of nurses, doctors, Physical therapists, Occupational therapists, Speech language therapists, and other medical professionals. The primary role of the social worker is to create a safe discharge plan in coordination with the patient, family, and interdisciplinary team.

Requirements

• Degree in Social work or related field of study
• 2 years' experience in Human Services or applicable field
• Prior experience in activities a plus
• Ability to manage moving priorities each day
• Strong verbal and written communication skills
• Computer skills for charting and documentation
• Strong team player with the ability to have FUN!

Responsibilities

• Completion of all required assessments, insurance reviews, and medical chart documentation
• Frequent interdisciplinary communication and meeting coordination
• Caseload management from admission to discharge
• Maintain effective, timely communication with patients and their families
• Understand and meet all government requirements for documentation
