This opportunity is full-time at 40 hours per week, offers full benefits. Wellington Regional Medical Center is located in Wellington, Florida. Requirements

• Prepared MSW or LCSW required.
• Past oncology experience preferred.
• 1 year acute hospital experience preferred.

Responsibilities

• Deal with the timely assessment of psychosocial issues of patients and their families/support systems.
• Advocate and coordinate a discharge plan ensuring appropriate utilization of hospital services and community services.
• Cover areas such as medication assistance, living wills, health proxies, and available literature and resources in the community as needs are identified.
• Maintain knowledge concerning regulatory standards (e.g., JCAHO, ACHA) and assist with keeping the department compliant.
• Provide assistance and expertise with disease support groups and programs within the hospital as needed.
• Recognize signs and symptoms of stroke and call a stroke alert overhead.
• Work with stroke patients and families for discharge planning and treatment based on needs, condition, and prognosis.
• Maintain currency in stroke knowledge by completing the annual 1 hour online training.
