About The Position

The Valley Health System has expanded into an integrated health network that serves more than two million people in Southern Nevada. Starting with Valley Hospital Medical Center in 1979, the Valley Health System has grown to include Centennial Hills Hospital Medical Center, Desert View Hospital, Spring Valley Hospital Medical Center, Summerlin Hospital Medical Center, Henderson Hospital, Valley Health Specialty Hospital and West Henderson Hospital. The position is responsible for the appropriate and timely discharge and transfer arrangements for patients referred to the Care Management Department.

Requirements

• Master's degree in social work required.
• At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed.
• Must have knowledge of social and physical factors that affect functional status at discharge.
• Knowledge of community resources to meet post discharge clinical and social needs.
• Currently licensed by the Nevada State Board of Examiners for Social Workers (LMSW).
• Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patients served on the assigned unit/department.
