Responsible for provision of social work services and discharge planning to patients of all ages while they are in the hospital for acute, outpatient, or emergency and rehabilitive care. Interacts with interdisciplinary discharge planning team, medical, and nursing staff. Maintains performance improvement within the department and participates in C. Q. I. Provides crucial psychosocial support, helping patients and families cope with illness, navigate the complex healthcare system, and plan for discharge by assessing needs, offering counseling, arranging resources (like home care, financial aid), coordinating care with medical teams, and advocating for patient rights, ensuring holistic well-being alongside medical treatment.
