About The Position

The Per Diem Social Worker (MA/MSW) / Case Management position at Corona Regional Medical Center involves providing direct and indirect social services for hospital patients, their families, hospital employees, and community members. Reporting to the Director of Case Management, the role includes working closely with case managers to address psychosocial issues, conducting medical record reviews for post-hospital planning, and performing assessments for unplanned readmissions and discharge planning. The social worker will collaborate with case managers to ensure a continuum of care through referrals to community agencies and assist physicians and hospital staff in managing patient care.

Requirements

• Master's degree preferred; Bachelor's degree in social work, psychology, or human development required.
• Minimum two years of social service experience in a medical setting or child/elder abuse program preferred.
• Two years' experience in an acute hospital setting preferred.
• Current BCLS (CPR) certification required.

Responsibilities

• Provide direct and indirect social services for hospital patients and their families.
• Work with case managers on daily management of psychosocial issues.
• Perform concurrent medical record reviews for post-hospital planning.
• Conduct social service case finding and intervention.
• Assess unplanned readmissions and complex/routine discharge planning.
• Act as a social services resource specialist.
• Collaborate with case managers to ensure continuum of care.
• Facilitate care through interaction with hospital departments and community services.
