About The Position

The Per Diem Social Worker (MA/MSW) / Case Management position at Corona Regional Medical Center involves providing direct and indirect social services for hospital patients, their families, hospital employees, and community members. Reporting to the Director of Case Management, the role includes working closely with case managers to address psychosocial issues, conducting medical record reviews for post-hospital planning, and performing assessments for unplanned readmissions and discharge planning. The social worker acts as a resource specialist and collaborates with hospital staff and community agencies to ensure a continuum of care for patients and their families.

Requirements

• Master's degree preferred; Bachelor's degree in social work, psychology, or human development required.
• Minimum two years of social service experience in a medical setting or child/elder abuse program preferred.
• Two years' experience in an acute hospital setting preferred.
• Current BCLS (CPR) certification required.

Responsibilities

• Provide direct and indirect social services for hospital patients and their families.
• Work with case managers to manage psychosocial issues and their resolutions.
• Perform concurrent medical record reviews for post-hospital planning.
• Conduct social service case finding and intervention.
• Engage in quality and risk management related issues.
• Act as a social services resource specialist.
• Perform unplanned readmission assessments.
• Conduct complex and routine discharge planning assessments.
• Collaborate with case managers to ensure continuum of care.
• Assist physicians and hospital staff in patient management.
