About The Position

This opportunity is full-time at 40 hours per week, offers full benefits. Wellington Regional Medical Center is located in Wellington, Florida. Requirements

• Prepared MSW or LCSW required.
• Past oncology experience preferred.
• 1 year acute hospital experience preferred.
• No travel requirements.

Responsibilities

• Timely assessment of psychosocial issues of patients and their families/support systems.
• Advocating/coordinating a discharge plan and ensuring concurrent appropriate utilization of hospital services.
• Providing information on medication assistance, living wills, health proxies, and available literature and resources in the community.
• Maintaining knowledge concerning regulatory standards (e.g., JCAHO, ACHA) and assisting with keeping the department compliant.
• Providing assistance and expertise with disease support groups and programs within the hospital as needed.
• Recognizing the signs and symptoms of stroke and calling a stroke alert overhead.
• Working with stroke patients and families for discharge planning and treatment based on needs, condition, and prognosis.
• Maintaining currency in stroke knowledge by completing the annual 1 hour online training.
