About The Position

The Social Worker - Level II position at NYC Health + Hospitals involves professional, supervisory, or administrative work in the operation of a social work department. This role provides psychosocial and other social work services to justice-involved patients across various Correctional Health Services (CHS) settings. The position aims to ensure patient-centered quality care and services, supporting better outcomes and improved patient experiences, in compliance with local, state, and federal regulations, as well as social work standards of care. This position is grant funded and requires working shifts from 12:00 A.M to 8:00 A.M.

Requirements

  • Valid license as an LMSW issued by the NYSED; or
  • Valid license as an LCSW issued by the NYSED.

Responsibilities

  • Screens medical records for historical and clinical information, conducts psychosocial evaluations through interviews with justice-involved patients and/or identified surrogates, and formulates appropriate social work treatment plans.
  • Works within various Correctional Health Services health care settings to provide individual and group counseling, as well as social services, as required.
  • Confers and consults with professionals in other disciplines to determine and implement a multi-disciplinary approach to patient care and develop treatment plans.
  • Provides outreach services, makes referrals to other governmental, public and/or community agencies, and prepares patients to accept services.
  • Documents and reports suspected cases of child abuse or neglect to the New York State Central Registry as a mandated reporter.
  • Appears and testifies at administrative and court hearings, as necessary.
  • Assists in plans for institutional or hospital discharge and evaluates alternate care arrangements for justice-involved patients.
  • Participates in staff conferences, special studies, committees, and quality assurance/performance improvement activities.
  • Prepares reports and keeps appropriate records.
  • Ensures compliance with relevant policies and procedures, national standards, and safety and quality management standards.
  • Mentors and assists less experienced social work staff in social work techniques.
