Requirements

  • Bachelor's Degree in social work
  • Licensed Social Worker in good standing in Minnesota
  • Two or more years of social work experience in a healthcare setting
  • Ability to communicate effectively in both written and verbal formats
  • Ability to follow all safety rules, regulations, policies, and procedures of the facility.

Nice To Haves

  • Master's Degree in social work
  • LGSW and ultimately LISW or LICSW license
  • Previous experience working with seniors
  • CPR certification

Responsibilities

  • Perform initial and ongoing assessment of the psychosocial status of terminally ill patients and families/caregivers.
  • Provide community resources, short-term anticipatory grief counseling, emotional support, and end-of-life education to patients and family/caregivers.
  • Assess the family/caregiver's bereavement risk and provide appropriate resources/interventions.
  • Monitor and assess family dynamics, caregiver ability, communication patterns, and safety issues.
  • Develop interventions based on assessment findings.
  • Participate in the development of individualized care plans involving patients and families.
  • Attend interdisciplinary group meetings to address psychosocial stressors.
  • Maintain clinical records on all patients referred to social work.
  • Assist with funeral arrangements and provide resources for financial, legal, and healthcare decisions.
  • Facilitate care conferences with hospice teams and families to ensure understanding of hospice services and the dying process.
  • Provide bereavement follow-up per agency protocol.
  • Assist with coordination of nursing home/residential hospice placements and respite stays.
