About The Position

A Social Worker, Bachelors provides social service and discharge planning functions for the hospital, provides resources for inpatients, outpatients, Emergency Department patients and support to the skilled unit designee when resources are unavailable, maintains performance improvement activities within the department and participates in CQI activities.

Requirements

• Bachelor's Degree required in Social Work, Psychology, Sociology or related field.
• 2+ years of professional social work experience.
• Previous experience in all aspects of social service and discharge planning.
• 1 year experience in a medical setting.

Responsibilities

• Provide social service and discharge planning functions for the hospital.
• Provide resources for inpatients, outpatients, and Emergency Department patients.
• Support the skilled unit designee when resources are unavailable.
• Maintain performance improvement activities within the department.
• Participate in Continuous Quality Improvement (CQI) activities.
