About The Position
This is a Full Time, salaried position at 40 hours per week on the Lake Forest hospital campus. Schedule is Monday through Friday, 8a-5p. In this role, the Social Worker would provide short-term counseling, assessment, care planning and coordination of psychosocial wraparound services to North Region outpatient psychiatry patients in helping them achieve optimal health outcomes. Responsibilities would also include clinical screenings/psychiatric diagnostic evaluations for new patient requests to establish care with our program. Remote work opportunities available.
