Where You’ll Work This full-time LMSW role is based at our Latham PACE Center, located on the beautiful, peaceful grounds of the Provincial House — a warm, welcoming space designed for creativity, teamwork, and participant-centered care. We also serve participants in Schenectady, providing ample variety in your day and expansion of your professional network. What You’ll Do As a Licensed Master Social Worker at Eddy SeniorCare PACE, you will: Provide Comprehensive Social Work Services Conduct psychosocial assessments for participants and families. Evaluate home environments and determine support needs. Deliver ongoing counseling, education, and crisis intervention. Support participants adjusting to illness, disability, or changing independence. Coordinate Across the Continuum of Care Participate in interdisciplinary care team meetings. Integrate your social work treatment plan into the participant’s overall Plan of Care. Manage transitions between home, ED/hospital, subacute rehab, respite, and more. Link participants with essential community resources and benefits. Requirements

  • Master’s Degree in Social Work (MSW) required
  • NYS LMSW license (or eligible)
  • Minimum 1 year experience working with older adults; home care experience preferred
  • Strong communication, assessment, advocacy, and crisis-intervention skill
  • Must have reliable transportation for home visits and meetings
  • Passion for working with frail elderly participants and supporting families
