About The Position

The P rogram of A ll-Inclusive C are for the E lderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Master Social Worker Primary Purpose: As a Master Social Worker, you will play a critical role in coordinating the social service programs at Senior Care Partners P.A.C.E., in line with our regulations and established policies and procedures. Your work will directly contribute to the well-being of our participants.

Requirements

  • Must possess a Master’s Degree in Social Work (MSW) and be licensed in the State of Michigan.
  • Minimum of one year of experience working with frail elderly or long-term care populations.
  • Detail-oriented with strong problem-solving abilities.
  • Excellent written and verbal communication skills.
  • Ability to maintain discretion and confidentiality.
  • Strong organizational skills and proficiency in MS Office Suite (Word, Outlook, PowerPoint, Excel).
  • Current CPR certification by AHA, or willingness to obtain it through Senior Care Partners P.A.C.E.
  • Demonstrate compassion, integrity, teamwork, and excellence.
  • Be a champion of the Senior Care Partners P.A.C.E. philosophy.
  • Provide exceptional support to all staff, external partners, and participants.
  • Ability to make independent decisions as circumstances warrant.
  • Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
  • Must meet CMS competency standards prior to working independently
  • Comfortable working in varied home environments and all weather conditions

Responsibilities

  • Conduct in-home comprehensive assessments and develop care plans.
  • Manage community resources effectively.
  • Collaborate with participants’ families and facilitate meetings.
  • Act as an expert in Advance Care Planning.
  • Provide education to alleviate caregiver burden.
